Asthma Review

A holistic and thorough remote review for asthma, incorporating NICE guidance. Patients can send videos of their inhaler technique and there are embedded videos for them to watch on the devices that they are using. This can allow your team to screen which patients are well controlled and don’t need a full review, and which need further input.

COPD Assessment Test (CAT)

The COPD assessment test (CAT) measures health-related quality of life. We aimed to systematically evaluate the literature for reliability, validity, responsiveness and minimum clinically important difference (MCID) of the CAT.

COPD Review

This is a comprehensive review for patients with COPD. The form incorporates all that is needed for QOF it also goes through everything that would be considered a gold standard review and complies with both NICE and GOLD guidance.

COVID Checkup (Oximetry & SATs monitoring)

This form supports the Oximetry@home programme to manage patients who have been diagnosed with COVID and are managing themselves at home, with an oximetry device

Diabetes Review

Every child and young person with diabetes is required to have an annual review once a year. The annual review provides an opportunity to take a look at all aspects of your diabetes, what is going well, what may not be going so well and to check for any early signs of other health concerns which may be related to it.

Dispensing Review Of Use Of Medication (DRUM)

The dispensing review of use of medicines (DRUM) is a requirement of the Dispensary Services Quality Scheme and is a review how a patient is using their prescribed medicines, looking at compliance and concordance.

This form can be used to obtain consent for flu vaccination in children prior to them attending.

Generalised Anxiety Disorder Assessment (GAD-7)

The Generalised Anxiety Disorder Assessment (GAD-7) is a seven-item instrument that is used to measure or assess the severity of generalised anxiety disorder (GAD). Each item asks the individual to rate the severity of his or her symptoms over the past two weeks.

Geriatric Depression Scale (GDS)

The Geriatric Depression Scale (GDS) is a self-report measure of depression in older adults. Users respond in a “Yes/No” format. The GDS was originally developed as a 30-item instrument.

Heart Failure Review

Developed in collaboration with colleagues in London this is a comprehensive review for patients suffering from heart failure. It takes account of symptoms of breathlessness, cough, fatigue and also screens for mental health issues.

Hypertension / Raised BP

This review allows patients to submit home blood measurements for review as well as conducting a thorough review for hypertensive patients, incorporating risk factor assessment. Patients can choose different durations of monitoring to submit and then return to the form each day to fill in their readings.

International Prostate Symptom Score (IPSS)

The American Urological Association (AUA) has developed the following questionnaire to help men determine how bothersome their urinary symptoms are and to check how effective their treatment is. This questionnaire has also been adopted worldwide and is known as the International Prostate Symptom Score (IPSS).

Learning Disabilities Annual Health Check

These annual health checks are for adults and young people aged 14 or over with a learning disability. An annual health check helps you stay well by talking about your health and finding any problems early, so you get the right care.

Long Covid Referral*

This form has been created to assist data capture of information needed to refer patients with long COVID symptoms. The form can be sent to patients prior to a GP appointment.

Mental Health Review

This is a comprehensive review for patients with Mental health diagnoses. Incorporating all that is needed for QOF it also goes through everything that would be considered a gold standard review.

New Patient Registration (GMS1)

Registration form for new patients (Adult).

Patient Health Questionnaire (PHQ-9)

The PHQ-9 is a valid self-administered scale that is useful for depression screening provided it is followed by a clinical interview to determine whether a patient truly has major depressive disorder when its results suggest that depression might be present.

Pill Check

Routine medical care for women taking the oral contraceptive pill includes annual check-ups with a doctor or nurse. During these check-ups, blood pressure and weight are measured.

Smoking and Weight

You can use this form to collect details about patients smoking status and current height/weight/BMI.
